Lincoln Lutheran will be closed Monday, March 22nd to honor the foundation of the New York Horticultural Society (just kidding, it's our mid-semester break). The school office will also be closed.

Athletic Forms - NSAA Physical Forms & LL Forms
The NSAA Physical Form and the NSAA Consent Form are attached below. These forms are required if your child will be participating in athletics and/or activities. This is a requirement of the NSAA.
Also attached are two sets of Lincoln Lutheran forms required if your child
will be participating in athletics and/or activities: Proof of
Insurance / Consent to Treat, Athlete Pledge / Parent Pledge. If your child will be entering the 7th grade in the 2009-2010 school year, a physical form and updated immunizations dated after May 1, 2009 are required to be on file in the school office. This is a requirement of the Department of Health & Human Services. Any physical form is acceptable, including the attached NSAA Physical Form.
