All 7th Graders Required to Have Physicals

Per Nebraska State Law, all 7th grade students are required to have a physical on file for their student records. If your 7th grade child will be participating in sports, and you already have the NSAA Physical Form completed, that form can be photocopied and will be all you need to meet this requirement.

If your 7th grader has not had a physical, you will need to make an appointment and submit a physical form that your doctor must sign before the start of the 2008-09 school year.For your convenience, the NSAA Physical Form is available in PDF format on our website. You can print this and take it to your doctors appointment.

If your 7th grader will be participating in sports at LL, they will be required to have this specific NSAA Form returned before any practice starts. If your 7th grader will not be participating in sports, a generic physical form from your doctor's office will be accepted per state law.

Again, all 7th grade students will be required to submit a physical form. The NSAA for is specifically required if they will be in sports, but any physical form from your doctor will be accepted as the 7th grade required physical.
